What to Bring to Your Appointment
To help us provide the most efficient care possible, please come prepared with the following:
- Completed new patient forms (sent to you in advance)
- Your dental insurance card or details
- A list of any medications you’re currently taking
- Recent dental X-rays (if taken within the last year)
- Any questions or concerns you'd like to discuss
Our team will also help you understand your out-of-network benefits if your plan isn't through Horizon BCBS NJ. We file all claims on your behalf and advocate for your reimbursement directly with your provider.

We are a fee-for-service practice, meaning we are only in-network with Horizon Blue Cross Blue Shield of New Jersey, but we are committed to making care accessible for all patients. If you have coverage through another provider:
- We will submit all claims to your insurance company for you
- Our team will help you maximize your out-of-network benefits
- Transparent treatment estimates will be provided before care begins

Do I need to have insurance to become a patient?
No. Many of our patients choose to pay out-of-pocket. Our team will discuss all financial options with you before starting care.
Will I get a cleaning at my first appointment?
In most cases, yes. If you are free of periodontal disease and have no complicating health issues, a preventive cleaning can often be completed the same day as your exam.
What if I have dental anxiety?
We offer sedation dentistry including nitrous oxide and oral conscious sedation to help you feel at ease during any procedure.
Can I transfer my records from another office?
Yes. Our team can request records from your previous dental provider — just let us know at the time of scheduling.
